Follow Me Home Program

When discharging a patient our “Follow Me Home” program promotes a smooth transition to independent living.

Life Supply offers a "Follow Me Home Program" where recipients are able to take home the equipment that they are familiar with and trained on in the facility. Through coordinated effort with our Intake department and your discharge planner a patient can leave the facility on their liquid portable and have their oxygen, hospital bed and wheelchair all ready for them when they arrive. We will work closely with your facility to have Oxygen and DME equipment in place prior to discharge. 

We have a full line of respiratory equipment and supplies, and we are a full Durable Medical Equipment (DME) company and at all times stock basic equipment such as walkers, hospital beds, commodes, bath aids, and what we don’t stock can be ordered.
